Interceptive Orthodontics for the Mixed Dentition and Teenager

the participants come from several province of Indonesia, to learning more about interceptive orthodontics. they are from Jakarta, Surabaya, Medan, Padang, Pekanbaru, Palembang, Banda Aceh and several region at Nangro Aceh Darussalam province also North Sumatera province like kak Riris,drg (Sibolga) and me (Tarutung, North Tapanuli). this seminar its very unique and give me more science and practice about patient management and also met kak Intan,drg (senior), dokter Rehulina (dosen Patologi Mulut) and bang Iskandar,drg,Sp.KGA (for sharing about pedodontics).

Dr Kenneth Lew is cited in the International Who’s Who (USA) as a ‘dental expert in the field of esthetic dentistry and orthodontics’ and has more than 20 years experience in those fields. Asked what motivates him, Dr Lew replied,’ I wanted to be a dentist. It was a clear choice over the Engineering scholarship in France I was offered after my ‘A’ levels. I loved the violin, studied under the well-regarded maestro Goh Soon Tieo but I knew, playing the violin well was good. But taking it to the next level needed not only sacrifice, but talent beyond measure of the ordinary man’.

With the support of a Public Service Commission scholarship, Dr Kenneth Lew received his Bachelor of Dental Surgery ( National University of Singapore, 1982). He set off with a National University of Singapore scholarship to obtain his specialist training, a Master of Dental Surgery (Orthodontics) from University of Adelaide, Australia in 1987. Dr Kenneth Lew modestly said,’ I am deeply honored to receive my FDSRCS from the Royal College of Surgeons, Edinburgh, without examination in recognition of my work. It has encouraged me to share my knowledge and do my bit by teaching other dentists what I have learnt over 2 decades of clinical dental practice’. Dr Lew conducts very well receives courses in Indonesia and Hong Kong teaching other dentists there.

Formerly the co-director of the Graduate Program in Orthodontics in NUS, Dr Kenneth Lew left academia for the challenges of private practice in 1994 to start Tanglin Dental Surgeons in Singapore. Dr Lew said,’ Clinical practice is emotionally rewarding. I have treated teenage patients with orthodontics, seen them marry. Now, I am treating their children!’

Dentistry is a major beneficiary of advances in technology, "Dr Lew said, It is immensely satisfying to be a dentist now." Dr Kenneth Lew is also one of the inaugural Invisilign^® and Simpliclear^® dentists in Singapore. We believe in giving choice to our patient.

As part of giving his patients the best looking smile and teeth we have started a Medical Aesthetics Division, in response to our patient's request for medical doctor administered procedures such as Botox and fillers.

Kecelakaan

Kecelakaan dpt menyebabkan trauma pd gigi n mulut. Trutama pd anak2, dikatakan hmpr 30% anak prnh mengalami trauma pd gigi n wajah pd saat bermain, berolah raga ato aktivitas lainnya.

Akibat kecelakaan pd jaringan mulut, gusi, bibir n pipi dpt berupa laserasi (luka robek) yg mengakibatkan perdarahan atau pembengkakan. Sedangkan pd jaringan gigi n tulang dpt menyebabkan avulsi gigi (gigi lepas), prerubahan posisi gigi, gigi patah atau tulang rahang patah baik rahang atas maupun rahang bawah. Bila keadaan ini tjd jgn lah panik, krn bila orang tua terlihat panik anak2 akan merasa khawatir n menangis serta tdk kooperatif.

Bila tjd avulsi (gigi lepas) :
Jika gigi lepas tsb tjd pd anak2 usia 5-6 thn, tdk perlu mrasa terlalu khawatir dgn keadaan tsb, krn gigi tsb akan tergantikan oleh gigi dewasanya kelak. Lakukan pemeriksaan k dokter gigi n bila perlu dilakukan rongsen foto shg dpt dilihat daerah dmn gigi tsb apakah ada patahan sisa akar atau kondisi lainnya.
Jika gigi dewasa yg lepas, biasanya tjd perdarahan pd keadaan ini bs dihentikan dgn meletakkan gumpalan kapas pd luka tsb lalu menyuruh anak mengatupkan mulutnya, selanjutnya :
1. Peganglah gigi pd mahkota gigi, jgn pd akarnya (krn hal tsb akan merusak sel2 yg diperlukan utk menempelnya gigi k tulang).
2. Cuci gigi dgn kucuran air bersih tapi jgn digosok.
3. Masukan gigi pd mulut diantara pipi n gusi utk menjaga tetap lembab atau bila memungkinkan bungkus dgn kasa bersih n masukan k dlm wadah yg diisi susu.
4. Sgera hubungi dokter gigi terdekat utk dilakukan penanaman kembali (replanted) n splinting (fiksasi gigi).
Faktor wkt sgt menentukan utk melakukan replanted. Jika ada bs bertemu dokter gigi dlm wkt ½ jam prognosisnya akan baik, jika lbh dr itu maka keberhasilan replanted akan berkurang antara 80-60%

Bila terjadi perubahan posisi gigi dari lengkung gigi
Lakukan reposisi dgn cara menekan dgn lembut utk mengembalikan gigi k posisi semula n katupkan rahang utk mcegah pergerakan gigi n sgera hubungi dokter gigi utk dilakukan splinting.

Bila Gigi Patah
Gigi patah bs berbagai macam kondisi mulai dr patah hny pd email sampai patah sluruh mahkota, berkumur dgn air hangat n segera hubungi dokter gigi utk dilakukan perawatan selanjutnya.

Bila tjd luka pd jaringan lunak :
Luka pd jaringan lunak dpt tjd pd gusi, bibir, lidah n pipi. Segera cuci n bersihkan luka dgn air hangat n hubungi RS terdekat. Utk mcegah perdarahan yg lbh byk pd luka, lalukan penekanan dgn kasa steril pd lidah, tarik lidah n lakukan penekanan dgn kasa steril

Rahang Patah
Biasanya tjd perubahan gigitan (mulut tdk bs dikatupkan n terbatas pd saat dibuka), ketidaksimetrisan wajah, pergerakan rahang yg abnormal, sakit pd saat rahang digerakan, pembengkakan, luka laserasi yg tjd pd daerah gusi n mukosa mulut, perubahan warna, hipersalivasi n halitosis (akibat berkurangnya pergerakan normal mandibula dpt tjd stagnasi makanan n hilangnya efek “self cleansing”) krn gangguan fungsi pengunyahan, Numbness (rasa baal), kelumpuhan dr bibir bwh, biasanya bila fraktur tjd di bwh nervus alveolaris. Lakukan kompres dingin dgn es yg dimasukan k dlm kantong plastik lalu segera hubungi RS terdekat.

Bila tjd trauma pd wajah, sebaiknya segera hubungi dokter gigi terdekat utk mdapatkan pemeriksaan lbh lanjut, rahang yg patah n tdk terawat dpt menyebabkan kelainan struktur yg lbh parah trutama pd masa pertumbuhan n perkembangan dpt menyebabkan cacat wajah sebab sendi temporomandibular joint (sendi yg mhubungkan tulang tengkorak n tulang rahang) merupakan pusat pertumbuhan pd wajah.

The sooner the better, dlm istilah medis tdp masa yg disebut golden period pd perawatan. Dmn pd masa itu, invasi bakteri blm menyebabkan infeksi n luka masih baru slain hasil perawatannya jg sgt baik jg mcegah kerusakan yg lbh parah atau kehilangan sruktur maupun fungsi
